Register your new bundle of joy

To register your baby, a newborn registration form must be completed by the principal (main) member within 60 days of the newborn's date of birth. The form must be accompanied by a copy of the birth certificate or a hospital confirmation of birth. If a newborn's surname differs from that of the main member, an affidavit confirming the difference in surname must be attached. If the newborn is adopted, adoption documents must be attached. These controls are utilised to ensure that there is no misuse of Scheme monies, i.e. member monies.
