Present your membership card at public hospital

• We issue a membership card to every main member when they join the Scheme and whenever they change their benefit
  option. If the member has registered dependants, we also
  issue a card to each adult dependant.
• We will issue extra cards for dependants should you request it.
• Only you and your registered dependants may use your membership card/s.
• If you remove a dependant, GEMS will issue you with a new membership card.

It is your responsibility to present your membership card to healthcare providers - such as doctors, dentists, pharmacists, specialists and hospitals - when you visit them. This includes your visits to public hospitals as these form part of the Designated Service Providers (DSP) for members on the Sapphire benefit option.

We encourage you and your dependants to present your membership card at all your visits to any of the public facilities for claims to be processed efficiently.
