The maternity care plan benefits cannot be altered. However, our Maternity Programme midwives will guide and support you with regards to the application of High risk benefits to ensure that you have sufficient consultations and procedures to ensure a healthy pregnancy.
Your midwife will guide and support you to access care and funding for high-risk maternity conditions to ensure that you have sufficient consultations and procedures to ensure a healthy pregnancy. If your pregnancy is classified as high-risk due to a history of a chronic condition you will be referred to the appropriate Chronic Disease Management Programme, for example: HIV, Asthma,Diabetes, and Hypertension. If your pregnancy risk is not covered by an existing maternity care plan and your normal scheme benefits have been exhausted, your Gynaecologist can apply for additional High-risk maternity benefits. Your Gynaecologist must complete a
PMB application form , email it to
[email protected] or faxed to 0861 004 367. To apply for additional High-risk maternity benefits (i.e. additional services like extra consultations, pathology or radiology tests), sections A, B, D and E on the PMB form must be completed by yourself and the treating doctor. The
PMB application form must be accompanied by a letter of motivation and other supporting information detailing your maternity risk condition. The application/motivation will be reviewed and the decision will be communicated to you and the healthcare practitioner.
