Momentum Medical Scheme
- Chronic Benefit - covers certain life-threatening conditions that need ongoing treatment. Chronic cover is unlimited according to the Chronic Disease List, which forms part of the Prescribed Minimum Benefits. Click here to read how to register a member for Chronic Benefits.
- Chronic Renal Failure Programme - the aim of this programme is to ensure that all patients who are at risk of developing renal failure or are already utilising the dialysis benefit have access to educational and lifestyle support.
- Organ Transplant Programme - a dedicated Transplant Programme Manager works closely with members and specialised clinical staff to ensure that members receive high quality, effective care for organ transplants.

- Maternity programme - to receive the full maternity benefit members must register between the 8th and 20th week of pregnancy.

- Chronic benefit programme - Covers certain serious and life threatening ailments that need ongoing treatment, available for members on the Health4Me Gold option.
- HIV benefit programme.
Note: The chronic benefit is limited to 26 conditions, Addison's Disease, Asthma, Bipolar Mood Disorder, Bronchiectasis, Cardiac Dysrhythmias, Cardiac Failure, Cardiomyopathy, Chronic Obstructive Pulmonary Disease, Chronic Renal Disease, Coronary Artery Disease, Crohn's Disease, Diabetes Insipidus, Diabetes Mellitus Type 1, Diabetes Mellitus Type 2, Epilepsy, Glaucoma, Haemophilia, Hyperlipidaemia, Hypertension, Hypothyroidism, Multiple Sclerosis, Parkinson's Disease, Rheumatoid Arthritis, Schizophrenia, Systemic Lupus Erythematosus and Ulcerative Colitis.
