Q: What is the process should I need to inform Momentum with regarding to any changes relevant to my practice information?
A: Please send the details to us via email at drnet@momentum.co.za or fax on (031) 580 0614. Please note that for any changes in banking details, we require the necessary documents to be posted to PO Box 2338, Durban, 4000. (Click here to access the change of banking details form).

Q: What is the process should I be interested in joining a Momentum provider network?

Q: Where can I obtain details of the different Momentum tariff reimbursement rates?
A: All the tariff rates are available under the menu “Important Information”, and then the sub menu “Rates” on the left hand side of the site. Select the applicable discipline to view the relevant rates

Q: What is the process for obtaining an authorisation for a patient or to register a patient on a disease management programme?
A: Contact us on 0860 11 78 59 for Momentum Health members, or 0860 10 29 03 for Momentum Health4Me members. The voice prompts will allow you to select “chronic registration”, alternatively hold for an agent.
